What this website collects
When you submit a form on this site (to contact us, apply to volunteer, or ask about sponsorship) we collect only what you type: your name, contact information, and the reason you're reaching out. The website has no user accounts and does not require registration for anything.
What we never collect here
Our website forms are not for medical information. Please do not include health details in any message. We discuss your health privately, by phone or in person at the clinic. Patient records at the clinic are protected under our Notice of Privacy Practices, available at the clinic.
How your information is handled
- Form submissions are delivered to clinic staff by a form-delivery service and used only to respond to you.
- Online donations and event tickets are processed by Givebutter, our secure giving partner. Your payment details go to Givebutter, not to this website; Givebutter's privacy policy governs that data.
- If we enable website analytics, it is basic, aggregate measurement of which pages are visited, not advertising tracking.
- We do not sell, rent, or trade your information.
