Certified Diabetes Educator

The Part time RN-Certified Diabetes Educator (CDE) works collaboratively as an active member of the inter-professional team to provide therapeutic patient and family centered care. This person provides education to individuals and families on a wide variety of topics with an emphasis on diabetes education. The CDE systematically and continuously collects and assesses data related to patient health status to develop, execute and evaluate the plan of care, establishes goals and strategies for meeting care coordination needs across settings., with the patient, family, and/or other care providers. The CDE considers factors related to safety, effectiveness and fiscal accountability in planning and delivering care. The CDE is a valuable part of MERCI Clinic. Must be able to supervise volunteers and meet guidelines and protocols established by the clinic and significant funding partners.

 

Education: Minimum-- BSN

Work Experience: Minimum-- 3-5 years ambulatory RN experience

Licenses & Certification: Minimum: Current Registration in NC, Certification by the National Board of Diabetes Educators

 

  Approximately 16- 20 hours/week. Must Work Tuesdays and Thursdays 8:00AM-5:00PM plus additional hours as needed.  

Pay is commensurate with education and experience.
